Unity is Northeast Wisconsin’s pioneering not-for-profit healthcare provider that offers a full collection of innovative care programs to improve quality of life. Programs include nonmedical home care, palliative care, hospice care, and grief support. Unity seamlessly adapts care with changing needs, circumstances, and wishes patients and families have in order to eliminate the anxiety and confusion of moving from provider to provider. Unity is the only locally owned and operated advanced illness provider in the region, ensuring seamless care in all settings. Established as the third hospice program in the nation, Unity launched Wisconsin’s first palliative care program in 2002, the area’s first hospice residence in 2007, the region’s only Grief and Education Center in 2021, and the area’s first pediatric hospice program in 2024.

Unique Programs

Advanced Cardiac Care – Holistic, at-home support for people with advanced cardiovascular diseases including heart disease, heart failure and stroke, and their families, to relieve symptoms, improve quality of life, and prevent emergency room and hospital visits
Compassionate Touch – Unity staff are specialty trained in compassionate presence and skilled touch techniques focused on the hands, back, and feet to help ease anxiety, pain, restlessness, agitation, and depression while strengthening trust between patients and Unity care team members.
Dementia Care –Specialized, holistic care and support to improve quality of life and promote a safe environment for patients and families facing dementia and Alzheimer’s.
Doorstep Delivery – Unity arranges for medications, medical equipment, and supplies to be delivered directly to the patient’s doorstep. Medicare or insurance covers the cost for those that are related to the patient’s life-limiting diagnosis.
Filled with Love – Keepsake bears and pillows handcrafted from the garments of hospice patients by Unity volunteers
Gifted Wishes – Makes end-of-life wishes come true for Unity hospice patients. Each granted wish holds a uniquely special place in the hearts of those who receive them.
Holistic Harmonies – A personalized, music playlist created for patients based on their preferences and converted to a QR code. The code enables patients, families, friends, and facility staff to easily access and play the music at any time on a digital device to boost mood, stimulate communication, and recall memories.
Paws of Comfort – Unity volunteer pet handlers provide guided interaction with their certified therapy dogs to bring joy to patients and reduce anxiety.
Reflections Journal Of Legacy and Life – A complimentary journal for patients to document and share their legacy of purpose, passions, and memories.
Respecting Valor Veteran Program - Specialized care and support for veterans includes custom care plans based on the branch and era of military service, as well as service related illnesses and conditions. Additional support includes Veteran to Veteran Volunteer companions to assist with life review and healing, pinning ceremonies to honor service and sacrifice, and connections to VAs and County Veteran Service Offices for veteran benefits.
Teleconnect – Virtually connects hospice patients and grief support clients to Unity care team members, as well as distant family and friends, to ensure timely care, peace of mind, and social interaction.
